Summary:
The Registered Nurse assesses and evaluates clients and oversees services as indicated by the Service Plan. This position organizes and oversees the initial and ongoing training program for Personal Care Assistants, as well as PCA competency assessments. The Registered Nurse is the team member responsible for developing and revising client service plans and conducting supervisory home visits to ensure quality care.
Supervisory Responsibilities:
• Traveling to clients' homes and managing their service plans. Develops and oversees PCA initial and ongoing training
• Listening to the concerns of family members and answering their questions.
• Educating caregivers and family on the aftercare or ongoing care of the patient.
• Providing suggestions for improved healthcare to staff and family members of the client.
Duties/Responsibilities:
• Organize and conduct an initial and ongoing training program for PCAs.
• Provide oversight for PCA competency assessments prior to performing tasks with clients. Ensuring any PCA wil not be assigned to care for a client with special conditions unless the PCA has received training and has demonstrated competency ni performing such services related ot such special conditions.
• Provide supervision for the provision of personal care tasks for clients determined to be medically frail or medically compromised.
• Complete client's service plans in accordance with agency policy and in coordination with the appropriate staff who will be providing the client's services.
• Review and revise service plans as needed.
• Conduct staff supervisory home visits ni the client residences according to agency policy, including an assessment of the client's general condition, vital signs, a review of the progress being made, the problems encountered by the client and the client's satisfaction with the services being delivered by the provider's staff. Supervision wil include observations about the appropriateness of the level of services being offered.
• Ensure proper documentation of training, client service plans, assessments, supervisory home visits and any other responsibilities assigned.
• Counsel PCAs and staff on difficult cases and potentially dangerous situations.
• Consult with the client and/or Authorized Representative in the event a medical issue arises.

Qualifications:
• Be at least twenty-one (21) years of age and possess a Bachelor's degree;
• Be able to communicate and understand return communication effectively in exchanges between the consumer, family representatives, and other providers, including the use of appropriate translator services as needed
• Be familiar with all applicable local, state, and federal laws and regulations concerning the operation and provision of home care services.
• Active RN license in the State of Georgia Physical Requirements:
• Must be able to lift up to 15 pounds at times.
• Bilingual Preferred but not Required.
